Your First Appointment

Prior to your first appointment, you will receive a β€œit’s time to check in!” text. Checking in prior toΒ your appointment will save you time when you arrive for the appointment. Before clicking the link to start the check-in process, please have the following available:

  • Driver’s license or a valid photo ID
  • Up-to-date insurance card
  • List of medications (if any)
  • Any past medical history and prior surgery information

When you attend your first appointment, please bring the following:

  • Driver’s license or a valid photo ID

  • Up-to-date insurance card

  • List of medications (if any)

  • Any X-rays related to the injury (please request disc from location X-rays were taken)

  • Any other tests, imaging or reports relevant to the injury (MRI reports, CT scans, office notes from PCP)

  • Referral Letter (if required)

  • Payment method if a co-payment is due at time of service
