Things to know about your upcoming appointment

You will be getting a text prior to your appointment telling you “It’s time to check in!” Checking in prior to
your appointment will save you time when you arrive on the day of your appointment. This system will
verify your date of birth. Before starting the check-in process, please have the following readily
available:

  • Photo ID
  • Up-to-Date Insurance Card
  • Medication List
  • Any past medical history and prior surgery information

Things to bring to your appointment:

  • Photo ID
  • Up-to-Date Insurance Card
  • Co-payment that may be due at time of service
  • Any X-rays related to this injury. Please request disc from location X-rays were taken
  • Any other tests, imaging or reports relevant to your injury (MRI reports, CT scans, Office Notes from PCP)
